State investigators say an 18-year-old apprentice miner died partly because of two violations by employees at an International Coal Group mine in West Virginia.
Office of Miners' Health, Safety and Training investigators said Tuesday that ICG has been cited for operating heavy equipment unsafely and improperly supervising Adam Lanham, who died in the May 30 accident. Lanham was run over by a scoop being used to haul gravel at ICG's Sentinel Mine in Barbour County.
Lanham, who worked for an outside contractor, had just 33 days of mining experience. Investigators say he was allowed to walk in front of the scoop by an ICG foreman who was trying to operate the equipment and supervise Lanham at the same time.
An ICG spokesman did not immediately respond to a request for comment.
